Please see the attached letter from the NHS immunisations team regarding this year's flu vaccine, which includes a link to an e-consent form.

The date of the session is Monday 25th November; when logging in to the e-consent form, please ensure you select the correct school (there are multiple schools with similar names).

Only ONE parent needs to complete the consent form per child to avoid duplication.

The deadline to book your child's vaccination is Tuesday 19th November; the portal closing date is FINAL – parents will need to contact the Immunisation Team directly after the closing date.
